Support Coordination (Level 2)
Our Support Coordinators work alongside you to bring your NDIS plan to life in a way that feels clear, empowering, and achievable. We take the time to understand what’s important to you and help you connect with providers who genuinely align with your goals, values, and needs.Beyond connecting you with the right supports, we help you implement your NDIS plan, navigate the NDIS system, and build the skills and confidence to manage your supports over time. We focus on capacity building, strengthening your ability to make informed decisions, coordinate services, and exercise choice and control — always honouring your dignity and independence.Through ongoing communication and collaboration, we make sure your supports work together seamlessly and adapt as your circumstances change. We identify and resolve barriers, monitor service delivery, and ensure your providers are meeting your needs. When things get tricky, we’re here to simplify the process, advocate alongside you, and help you stay on track. We also support you to prepare for NDIS plan reviews, gather evidence, and understand what’s working well and what may need adjustment. Our goal is to help you feel confident, connected, and supported as you work towards greater independence.
